Grant Report Forms
If you have received a grant from the Foundation, we are interested in learning about the results of your work. This is one of the most important ways the Foundation can learn about the effect our investments are making throughout the county.
Your evaluation report deadline is generally 30-60 days after the end of your grant period. See your original award letter for specific reporting instructions.
We rely on your candid grant reports to assist us. Please provide your honest thoughts about the short and long-term results of your project. We are just as interested in what did not work as what did. While we understand the wish to focus on success, we also respect the candor and insight in sharing lessons learned.

Submitting the Grant Report
Report submission by email is required. Completed reports should be sent electronically to reports@sbfoundation.org. Please type your organization name and grant type in the subject line.
