How to Submit a PDF Final Report

  1. Please download the appropriate grant report form from the Grant Report Forms page and follow the instructions carefully, as they vary per grant program.
  2. Save the report to your desktop before completing, and open the forms in Adobe Acrobat Reader or another PDF reader. If you open the forms in your web browser edits may not be saved correctly. You can download Adobe Acrobat Reader for free from
  3. Include the name of your organization, year grant was received, and “[org name] [grant year] [grant program] Final Report” in the email subject line. (Example: My Nonprofit 2021 Towbes Fund for the Performing Arts Final Report)
  4. Completed reports should be emailed to by the deadline noted in your award letter.
